New location for Care Plus Clinic patients:
From 15th March 2010 the following clinics will be located at:
2nd Floor, Work & Income building, 49 Lincoln Road, Masterton
- Wellness Clinic
- Care Plus Clinic
- Diabetes Clinic
- Respiratory Clinic
- Cardio Vascular (CVD) Clinic
Parking will be available for patients of these clinics behing the Work & Income building. Drive through the front car park off Lincoln Road around to the back of the building.

Our Care Plus is a Primary care Initiative funded by the Ministry of Health, designed to enhance care for patients who have high health needs. This programme is designed to reduce visits to the doctor by managing chronic illnesses such as Asthma, Diabetes and Obesity more efficiently
Masterton Medical has a full time Nurse running the Care Plus programme. The programme is designed to provide shared care by the patient’s GP and Care plus nurse.
Goals are set and a care plan is developed for each patient. A partnership is encouraged where the patient and health team work to achieve the health goals.
There is a strong focus on education and preventative care.
Each patient has four designated appointments, three months apart, throughout the year. which are subsidised.
