Chapel Street Family Doctors patients transferring to Masterton Medical will begin coming to clinics here on 12th April 2010.
Parking will be provided for you at the Work and Income building. This will be clearly signposted. A lift operates to the 2nd floor. We ask that mobility scooters are left on the ground floor.

Our Care Plus is a Primary care Initiative funded by the Ministry of Health, designed to enhance care for patients who have high health needs. This programme is designed to reduce visits to the doctor by managing chronic illnesses such as Asthma, Diabetes and Obesity more efficiently.
Masterton Medical has a full time Nurse running the Care Plus programme. The programme is designed to provide shared care by the patient’s GP and Care plus nurse.
Goals are set and a care plan is developed for each patient. A partnership is encouraged where the patient and health team work to achieve the health goals.
There is a strong focus on education and preventative care.
Each patient has four designated appointments, three months apart, throughout the year. which are subsidised.
